If you search for a passport application form, you usually find two names right away: DS-11 and DS-82. They are not interchangeable. Choosing the wrong form can waste time, delay mailing, or force you to start over.

This guide keeps it simple. Here is what each form is for, what documents usually go with it, and where applicants often run into trouble before the packet is even reviewed.

Which passport form you need

The short version is this: DS-11 is commonly used for first-time applicants, children, and people who cannot renew by mail. DS-82 is the standard renewal form for eligible adults.

Before you print anything, ask yourself three questions:

  • Is this your first US passport?
  • Are you renewing an adult passport that is still eligible for mail renewal?
  • Do you still have your most recent passport in acceptable condition?

Those answers usually tell you which path to follow.

When to use DS-11

Use DS-11 when you are applying for a first passport, replacing a lost or badly damaged passport, applying for a child, or renewing in a situation that does not qualify for mail renewal.

DS-11 applications are generally submitted in person. That means you should expect an appointment at an acceptance facility such as a post office, clerk of court, or other passport office partner.

For many people, DS-11 takes longer simply because there are more moving parts: appointment scheduling, identity checks, original documents, fees, and photo review all happen in a more formal workflow.

When to use DS-82

DS-82 is the form most adults want if they qualify. It is designed for passport renewal by mail and is usually the smoother option because you skip the in-person acceptance step.

You typically use DS-82 when your previous passport was issued in your current name or you can document the name change, the passport was issued when you were old enough to qualify for adult renewal rules, and it is still in a condition the authorities accept.

Even when DS-82 is the right form, small errors still cause delays: outdated signatures, mismatched names, missing payment details, or a photo that does not meet requirements.

Documents to prepare

Most applicants should prepare the same core pieces before completing the form:

  • the correct form version, filled out clearly and consistently
  • proof of identity or your previous passport, depending on the case
  • any name-change documentation if needed
  • payment in the accepted format
  • a compliant passport photo

If you prepare these items first, filling the form becomes much easier because you are not guessing at dates, document numbers, or mailing details.

Photo mistakes that slow the application

Applicants tend to focus on the form and forget the photo, but the photo is often the simplest place to lose time. Wrong size, shadows on the background, low resolution, heavy edits, glare on glasses, or poor head positioning can all cause problems.

If you want to reduce the risk, prepare the photo separately before sending the packet. With IDPhotoBy, you can make a digital passport photo that is cropped and checked to match common US passport standards before you print or attach it.

The form matters, but the cleanest applications usually come from people who handle the photo step early instead of treating it as an afterthought.
